Tdap Booster Shot

         

Parents of 5th Graders

  • Your child is required to have a Tdap booster before starting 6th grade.

  • These shots may be obtained through your family doctor, military clinic, or Chesapeake Health Department.

  •  This shot is free if obtained through the health department.

  • Please bring a copy of the shot record after your child gets their shot to the school nurse.

  • If you receive your shot over the summer, take a copy directly to the middle school.
