Digital Mammogram Programs for Employees
Chesapeake Public School offers you your choice of one of the following:

Sentara Leigh Hospital has been providing the mobile mammogram
screening for Chesapeake School employees for many years.
This year, they have a new mobile digital mammography van.
Their scheduling procedure will be the same this year as
in past years. The participant must call 1-800-SENTARA
(1-800-736-8272) or 455-7500, Monday through Friday, 8:00a.m.
to 6:00p.m. to schedule her mobile digital mammogram appointment
for one of the locations listed below:

PLEASE NOTE: There are no co-pays for routine preventive mammogram screenings with the Anthem KeyCare PPO an HealthKeppers HMO plan through
Chesapeake Public Schools. Referrals for ths procedure are not required from your primary care physician (PCP) if you are enrolled
in the Anthem HealthKeepers HMO plan. If you received a mammogram last year, please schedule your appointment during the same month as last year's screening.
Please remember that this screening is for your routine check-up. If you have health problems with your breast, breast implants, or a personal
history of breast cancer, any mammogram should be coordinated through your physician.
- Chesapeake Regional Medical Center, The Breast Center, is also offering digital breast imaging for Chesapeake
Public School employees on a year round basis, during hours that are convenient for
those who work in a school system. This program is offered
through Chesapeake Regional Medical Center. They do not
offer the Mobile Mammogram.
To schedule your DIGITAL mammogram, fill out the registration form and the
medical release form and fax them to the numbers indicated on the forms. The Chesapeake Regional Medical Center will call you at the number you indicate to schedule your appointment. You may also call the Outpatient Scheduling Department at 757-312-6137. Be sure to indicate that you are a Chesapeake Public Schools employee.
