📝 Participation Checklist
To get your student on the field, please make sure the following are in order:
Signed Permission Slip: A completed and signed form must be turned in prior to the first game. Printable Intramural Permission Slip.
Transportation: Please ensure your student has a guaranteed ride home immediately following the conclusion of the games.
