Transition Services

The Code of Virginia, section 22.1.217.3 requires that each high school must publicly identify a faculty member responsible for special education transition planning and coordination on the school's official website.  

This individual would be a faculty member who can assist in the:

  • dissemination of designated transition information and resources,

  • coordination of designated transition meetings with transition specialists, programs, and activities,

  • provision of school-requested transition resources and materials to staff,

  • attendance at designated transition meetings and informational sessions; and 

  • serve as a building liaison for the Department of Aging and Rehabilitative Services (DARS) and other transition-related vendors.

The Department of Exceptional Learning has identified that this is a long-standing practice in existence in our district.  Your selected Special Education Department Chairpersons execute this role annually with collaboration and communication with your school's assigned transition specialist.
